The Gentle Journey Quality of Life Framework™

A clearer way to recognize changes in quality of life—helping veterinary teams and pet parents recognize what matters earlier and make more informed decisions.

Why This Framework Uses Four States—Not a Score

A pet may still eat, seek affection, walk, or enjoy favorite activities while experiencing significant problems in another area. Those preserved abilities matter—but they shouldn’t cancel out signs of pain, decline, or distress.

The Gentle Journey Quality of Life Framework™ evaluates each area independently rather than adding preserved abilities together to produce a score, helping reveal whether a pet is Doing Well, Managing/Coping Well, Slipping, or experiencing Distress/Suffering.

The Four Quality-of-Life States

The Gentle Journey Quality of Life Framework™ uses four states to describe a pet's current quality of life: Doing Well, Managing/Coping Well, Slipping, and Distress/Suffering.

Doing Well

Your pet is comfortable and able to enjoy the activities, interactions, and routines that are important to them, without significant limitations from pain, illness, or declining function.

Slipping

Your pet is having increasing difficulty adapting to changes caused by illness, pain, or declining function. Signs once seemed manageable are becoming more frequent, significant or disruptive to their comfort and daily life.

Managing/Coping Well

Your pet has noticeable changes or limitations but is adapting and remains comfortable. They can still engage meaningfully with life, even if activities need to be modified or supported.

Distress/Suffering

Your pet is no longer adequately coping. Pain, breathing difficulty, anxiety, weakness, restlessness, or other effects of illness are causing significant distress or preventing them from being comfortable.

The goal is to maintain Doing Well or Managing/Coping Well, recognize Slipping early, and prevent Distress/Suffering whenever possible.

Go Beyond the Framework

The Framework gives you a clear way to recognize changes in your pet’s quality of life. But a PDF can only go so far—and there is so much more I want you to understand.

The Gentle Journey Quality of Life Course™ is a five-part, nearly 90-minute video series covering the guidance I would want every pet parent to have after a quality-of-life consultation: how to recognize meaningful changes, understand what your pet may be experiencing, and make difficult decisions with greater clarity.
